Molten Metal Spill Claims at Least 8 Lives Following Massive Ladle Explosion

In one of the most catastrophic industrial disasters to strike Andhra Pradesh in the last decade, at least eight workers-with local reports indicating the toll may have risen to nine-were killed and several others critically injured after a massive explosion at the Visakhapatnam Steel Plant (Rashtriya Ispat Nigam Limited) on Monday afternoon.

A heavy industrial ladle carrying tons of liquid steel, superheated to approximately 1,600°C, snapped and spilled its contents across the shop floor, leaving the workers stationed underneath with no chance of escape.

The Incident: A Breakdown of the Disaster

The disaster occurred at approximately 4:15 PM on June 8, 2026, inside the Steel Melting Shop-2 (SMS-2) and Caster-2 facility. Here is a clear timeline of how the tragic events unfolded:

  • The Precursor: Casting operations were actively underway at the facility. Workers were preparing to open the slide gate to pour hot crude steel from a massive overhead ladle into a tundish for further casting.
  • The Explosion: Without warning, a severe and sudden explosion took place. Eyewitnesses and rescued workers reported hearing a deafening blast-suspected to be the structural snapping of the overhead crane holding the vessel.
  • The Aftermath: The mechanical failure caused the ladle to plummet to the ground, unleashing roughly 150 tonnes of boiling, liquid metal across the factory floor. A massive ball of fire immediately shot up to the ceiling, igniting the overhead crane and trapping the personnel stationed below.

Trade union leaders noted that while a few workers had fortuitously stepped away for a brief break moments before the blast, those directly underneath the ladle were caught in the immediate blast radius. Fire tenders and emergency personnel battled hazardous conditions and intense heat for nearly 90 minutes to douse the flames and retrieve the victims.

The Victims: Lives Lost and Injured

The devastating spill resulted in immediate fatalities, with victims suffering horrific 100% burns. The deceased comprise both permanent RINL employees and contract labourers.

  • Deceased Permanent Staff: K. Prabhakar (General Foreman), Gold Kumar (Manager), Gontina Bhanu Kumar (Technician), G.V. Appa Rao, and M. Krishna Nagu.
  • Deceased Contract Workers: Ramana, Trinadh, and N. Appala Raju.
  • The Injured: At least six workers sustained severe burns ranging from 40% to 90%. They were given first-aid at the steel plant hospital before being shifted to specialized private facilities, including KIMS and Seven Hills Hospital, as their conditions rapidly deteriorated.

Government Response and Relief Measures

The tragedy has drawn immediate responses and condolences from the highest levels of the state and central governments:

  • Prime Minister’s Office: Prime Minister Narendra Modi expressed deep sorrow over the loss of lives and announced an ex-gratia payment of ₹2 lakh from the Prime Minister’s National Relief Fund (PMNRF) for the families of the deceased, alongside ₹50,000 for the injured.
  • State Leadership: Andhra Pradesh Chief Minister N. Chandrababu Naidu expressed his shock and ordered district authorities to coordinate swift rescue and relief operations. State Home Minister Vangalapudi Anitha cut short her scheduled visits and rushed to the site to personally oversee the emergency efforts.
  • Opposition: Former Chief Minister Y.S. Jagan Mohan Reddy demanded a comprehensive, transparent inquiry into the safety lapses, emphasizing that industrial safety protocols and worker lives must never be compromised.

Investigations and Historical Background

Following the accident, the Union Ministry of Steel ordered the immediate suspension of operations in the affected unit. To ensure absolute transparency, the Ministry has constituted a three-member external inquiry committee, headed by the Director-in-Charge of the Bokaro Steel Plant (SAIL), to conduct an independent probe into the mechanical and safety failures that triggered the blast.

Local trade union leaders have raised serious allegations against the plant’s management, claiming that the facility has been suffering from severe staff shortages-with over 10,000 workers retiring or facing removal in the last three years. They allege that this manpower crunch has led to dangerous compromises in critical equipment maintenance.

This tragedy serves as a grim and painful reminder of a similar disaster at the Vizag Steel Plant on June 14, 2012, when 16 workers were burnt alive following a massive explosion at a newly commissioned oxygen plant.
